Methodological challenges designing pragmatic, multi-centre randomised controlled trials in critical care

چکیده
Population Unlike many other clinical areas, patients admitted to intensive care units (ICUs) do not have easily identifiable diseases (e.g. diabetes or cancer), but are classified as having syndromes with hazy definitions that may be inconsistently applied both within and between centres. The definition and implementation of inclusion and exclusion criteria is therefore difficult, resulting in clinically heterogeneous populations.

Pragmatic trials in primary care. Methodological challenges and solutions demonstrated by the DIAMOND-study
BACKGROUND Pragmatic randomised controlled trials are often used in primary care to evaluate the effect of a treatment strategy. In these trials it is difficult to achieve both high internal validity and high generalisability. This article will discuss several methodological challenges in designing and conducting a pragmatic primary care based randomised controlled trial, based on our experienc...

BACKGROUND Successful recruitment of participants to any trial is central to its success. Trial results are routinely published, and recruitment is often cited to be slower and more difficult than anticipated. This article reflects on the methodological challenges of recruiting women with prolapse attending United Kingdom (UK) gynaecology outpatient clinics to a multi-centre randomised controll...
متن کاملUndertaking a randomised controlled trial in the police setting: methodological and practical challenges
BACKGROUND There has been an increased drive towards Evidence Based Policing in recent years. Unlike in other public sector services, such as health and education, randomised controlled trials in the police setting are relatively rare. This paper discusses some of the methodological and practical challenges of conducting a randomised controlled trial in the police setting in the UK, based on ou...
متن کاملA pragmatic multi-centre randomised controlled trial of fluid loading and level of dependency in high-risk surgical patients undergoing major elective surgery: trial protocol
BACKGROUND Patients undergoing major elective or urgent surgery are at high risk of death or significant morbidity. Measures to reduce this morbidity and mortality include pre-operative optimisation and use of higher levels of dependency care after surgery. We propose a pragmatic multi-centre randomised controlled trial of level of dependency and pre-operative fluid therapy in high-risk surgica...
متن کاملThe LIPPSMAck POP (Lung Infection Prevention Post Surgery - Major Abdominal - with Pre-Operative Physiotherapy) trial: study protocol for a multi-centre randomised controlled trial
BACKGROUND Post-operative pulmonary complications are a significant problem following open upper abdominal surgery. Preliminary evidence suggests that a single pre-operative physiotherapy education and preparatory lung expansion training session alone may prevent respiratory complications more effectively than supervised post-operative breathing and coughing exercises. However, the evidence is ...
